Part-time Case Manager

Job Description: Case Manager

Salary $26.00/hour

The Coalition for the Homeless is seeking a part-time detail-oriented and experienced Case Manager to join our team. This is a contract position, contingent upon the renewal of funding through the Community Partnership for the Prevention of Homelessness.

This exempt professional position is based at the Coalition for the Homeless Case Management Division. This individual will be responsible for the delivery of social services to singles. The incumbent will become a member of a case management team seeking to provide comprehensive and integrated social services to program participants.

Key Responsibilities:

1. Orients the participants to the social services that will be provided through the employment services program.

2. Conducts intake assessments on all program participants where case manager is needed.

3. Identifies problems of participants, documents, evaluates and recommends the necessary services to be provided. Caseworker will identify the appropriate agencies to assist the participants, arrange transportation and do periodic follow-up. Will assist the participants with obtaining food stamps, Medicaid, SSI and other entitlement when appropriate. Maintains confidentiality of all participants files and interview sessions.

4. Participates as part of a case management team to discuss progress of participants in the employment services program.

5. Holds small discussion groups/activities on various topics in order to help the participants to cope with their problems, address the possible resolutions and provide incentive to change the participants way of life and also to promote appropriate socialization.

6. Implements case management model with participants which includes development of case assessment, case plans, documentation of counseling sessions, affects necessary referral and follow-up.

7. When appropriate, attempts to identify and communicate with family and/or friends for reunification.

8. Collaborates with other team members to ensure comprehensive care. Convenes and facilitates team meetings when appropriate; actively participates as a member of the team on other cases.

9. Cooperates with other staff people in an effort to assist the participants in leaving the facility and becoming independent.

10. Acts as a liaison between the participants and the available community service; maintains network with other social services and resources.

11. Counsels each participant on caseload at least twice a month.

12. Maintains and updates all monthly data and logged information; submits monthly report by the fifth of each month along with any documentation as requested.

13. Participates in all staff development sessions related to social service delivery; participates in all case management meetings.

14. Other duties may be assigned by the supervisor.

KNOWLEDGE AND ABILITIES REQUIRED

1. Bachelor’s in social work or human services.

2. At least one year of case management experience.

3. Understanding of and ability to effectively relate to persons of diverse social and cultural backgrounds, with a growth-oriented approach to service.

4. Strength-based or growth-oriented approach to service delivery.

5. Excellent writing and communication skills.

6. Ability to function as a team player and to work closely with staff and heads of household participants.
